Acetylcholine activates muscles, so blocking it causes muscle relaxation and paralysis. Botox injections place botulinum toxin directly into a specific muscle. There it has a direct effect on acetylcholine in nerve synapses, blocking signals that would normally cause the muscle to contract.
Neurogenic detrusor overactivity (an overactive bladder disorder caused by a dysfunction in the nerves that supply the bladder) in pediatric patients 5 years of age and older who do not respond to, or are intolerant of, anticholinergic medication.

In general, the initial effect of the injections is seen within three days and reaches a peak at one to two weeks post-treatment. Each treatment lasts approximately three months, following which the procedure can be repeated. At repeat treatment sessions, the dose may be increased up to two-fold if the response from the initial treatment is considered insufficient, usually defined as an effect that does not last longer than two months.

The risk of symptoms is probably greatest in children treated for spasticity but symptoms can also occur in adults treated for spasticity and other conditions, and particularly in those patients who have an underlying condition that would predispose them to these symptoms.
